
Required Documents Form
IMPORTANT: The following are documents that will be needed in order to process your application for the Social Services Office to determine your eligibility. Please submit to the attention of: Social Services Office (954) 597-3626. Please utilize this checklist to organize your documents.
--All household income documents, including: Last three (3) current pay stubs., Child Support, Alimony, Unemployment Benefits, Pension, Social Security, Social Security Disability, Food Stamps, Public Assistance, Family Assistance
--If applicable: Proof of Employment Layoff or Termination
--Proof of Residency—picture ID, rental agreement, FPL or water bill, voter’s registration.
--Documentation of all current Household Expenses, with current cancelled checks to show last payment made. Expenses include:
--Mortgage
--Rent ( must show rental agreement document) If name is not on lease, must have letter of support from person named on lease.
--Electric
--Water/Sanitation
--Cell Phone ( internet services excluded)
--Car Payment
--Car Insurance
--Credit Card Payments
--Food Stamp Statement
--Gas (estimate monthly amount)
--Child Care
--Last year’s tax return (2023). Proof of refund expense.
--Last 3 months bank statements
--Medical Payments/ Prescription Payments
Applications cannot be processed without all of the above documentation. A separate financial application will be required and eligibility will be determined by Social Services.
